Review Your Medication List Before Leaving the Hospital
When doctors say you can go home from the hospital, they give you a long list of instructions . They may forget to advise you on which home medications you should stop. Before you (or a family member) leave the hospital, read this list of medications you take regularly and as needed.

During any hospital stay, many home medications may be canceled or changed. Common examples include medicines for blood pressure and diabetes, which will be adjusted in response to fluctuations in these parameters during illness. In addition, new medications will be prescribed at discharge.
